Visiting Students Application Process

Visiting student applications for rotations occuring between
June 2 - March 31 will be accepted beginning March 3 for the 2008-2009 academic year.

Students must submit a completed application packet to the Pritzker School of Medicine at least six weeks prior to the start date of their desired rotation.

Rotations will be given on a first-come, first-served basis to students who submit a complete application. Calling to check availability will not guarantee a spot in a desired rotation.

Applications will not be reviewed until the following items have been submitted in their entirety:

  1. Visiting Student Application (PDF)
  2. Student Photo (1"5/8 x 2"2/8)
  3. Letter of good-standing (no more than one page in length) from medical school with school seal
  4. Current hospitalization insurance coverage*
  5. Current malpractice coverage (not less than $1 million per incident/$3 million aggregate) OR Proof of Indemnification (PDF) (must have school seal)*
  6. Completed Immunization Form (pdf) with proof of immunity to measles, mumps, ruebella, hepatitis B, varicella, tetanus and tuberculosis certified by a health care official and attached lab reports
  7. Proof of HIPAA compliance

* Students will not begin clinical work without proof of health insurance and proof of liability insurance.
