Clinic Details

Address: 5350 S Prairie Avenue at Chicago Youth Programs (Washington Park Neighborhood)
Clinic Operational Hours: 5:30-8:00pm on Tuesdays (weekly)
Patient Population: Pediatric patients primarily seeking school physicals and vaccines. Max of 10 patients per night, with patient check-in closing at 7pm.

Required Documentation

If this is your first time volunteering, you will need to email the following required documentation to WPC's Physician Coordinator, Samuel Martin Samuel.Martin@uchicagomedicine.org.

Copy or photo of your license to practice independently
Completed copy of the Faculty Volunteer Application

Arrival Information

Street parking is available outside the clinic. Plan to arrive at 5pm to prep for clinic. If you are running late, please let us know! Contact Samuel Martin - Physician Coordinator at Samuel.Martin@uchicagomedicine.org.

Clinic Workflow

First-year medical students are paired with third- and fourth-year medical students or residents. They will conduct the initial interview and examine patients.
The medical students will then present to the attending physician, and will discuss the collection assessment and care plan with the patient.
1. There are three curtain-separated exam tables and can see up to a total of 10 patients per night.
